Granted, the quality is usually not as good as something professionally printed, but it'll do the trick if you're really keen on a specific design you can't find in the t-shirt colour you want.
Also, I believe spreadshirt.com will let you make your own t-shirt designs, and you can choose different t-shirts in different colours.
I, personally, sometimes fabric-paint my own geek t-shirts, but it requires basic painting skills and a transfer pencil if you don't wanna draw it on the t-shirt by hand. I've made Sherlock and Cabin Pressure t-shirts that way, and I'm thinking about a Dd one, but I haven't really decided on a design yet.
